The Program will include the following:

  • Bi-weekly 50 minute appointments that will include the clients progress and weekly recommendations with a full set of notes.
  • Goals set by the client/coach at the beginning of the program to help client implement life style habits in order to achieve overall success.
  • Tools to implement healthy lifestyle habits that will allow your to reach and maintain your desired goals.
  • A variety of handouts, recipes, books, CDs, food samples, food demo’s, store tours and other materials.
  • Weekly support via e-mail, text or phone

You will learn ways to help yourself achieve a healthier diet and lifestyle. Please read the following. If anything is unclear, please ask.

 

This Agreement is made today between the Coach of the Program and the person named at the end of this document, [the Client]. The Program in which you are about to enroll will include the following:

 

A.    Two 50-60 minute appointments each month for six months, which will include discussion of your progress, recommendations and a full set of notes.

 

B.    Tools to implement healthy lifestyle habits that will allow your to reach and maintain your desired goals.

 

C.    A variety of handouts, recipes, books, CDs, food samples, food demo’s, store tours and other materials.

 

D.    Weekly support via e-mail, text or phone.

 

E.    Goals set by the client/coach at the beginning of the program to help client implement life style habits in order to achieve overall success.
